Patient Forms
Once you have scheduled your appointment, we will need certain information from you as a new patient at Gastroenterology Associates of Southwest Florida including details about your current gastrointestinal problem, your medical history, and insurance information.
For your convenience, you will receive an email or text with a secure link to check in online from home three days prior to arriving to the appointment. This will help to speed up our patient registration process. If you are unable to receive email or text please arrive 30 minutes prior to your appointment to register and complete the check in process.

Your First Appointment
Please check your email or text for the link to complete your registration and check in online prior to arriving for your appointment. If you are unable to complete this prior to arriving you should arrive 30 minutes early to your appointment in order to complete the electronic registration and check in while in our office. You should bring the following with you:
- Your patient insurance card
- Photo ID
- A list of current prescriptions or over-the-counter medication, including dose and frequency
- Information about your medical and surgical history
- Recent test results, x-rays, or relevant records
Insurance and Payment Information
Gastroenterology Associates of Southwest Florida participates in Medicare and most major insurance plans. Our practice will file insurance claims on behalf of our patients; however, patients are responsible for deductibles, co-insurance, co-payments, and non-covered charges.
